Like Marcelo, I think people are hung up on having the leg from your telephone go over an IP network. There are times when there is value in doing that, for instance, you have a WiFi-enabled mobile and you're in a foreign country and don't want to pay the insane roaming charges you're likely to pay by using GSM. However, I don't think this kind of setup is a requirement for true VoIP be it on the iPhone ...
